We realize that filling out this form requires some ministry decisions, which requires very prayerful consideration. Please be assured of my personal care and concern for you as you seek the leading of the Lord.

* Please note: Licensed Ministers and Ordained Ministers with less than 25 years experience in approved ministries, are subject to time limits of inactivity when they leave that approved ministry. For Licensed Ministers it is one (1) year; for Ordained Ministers it is three (3) years of inactivity, at which time the Denominational Office may ask you to place your credentials on file until you re enter an active, approved ministry. This is not a question of your calling, but is about your activity and need for credentials. Please call the D.S., if you have any questions.
